Group Enrollment Instructions and Templates for Horizon

This page includes templates and instructions for uploading enrollment files to the Group Portal.

Overview

One feature of the Group Portal is the ability to add members to your plans in bulk. You can do this by filling out a spreadsheet with your members and uploading it to the Group Portal.

There are separate spreadsheets for HSAs, FSAs, and HRAs. Choose the product you want to enroll members into, find the spreadsheet from the list below, and enter the employee information into it. 

To do this, log in to the Group Portal, go to the Add Members page, click Upload File, and enter your spreadsheet.
